to become a subcontractor with us, please complete the following steps:
- Submit a Signed W9 Form
- Submit a Certificate of Insurance meeting Empire Contractors' Insurance Requirements
Please either fax the required forms to our office, or submit via email to prequal@empire-contractors.com
The following scopes of work do not need to submit a prequalification form
- Testing Agencies
- Environmental Services
- Architecture/Engineering Firms
