  1. What all needs to be submitted with my contract?
    • Signed subcontract
    • Attachment "B" for the overall job estimate
    • Job specific certificate of insurance
  2. What all needs to be submitted with my application for payment?
    • Attachment "A" - Subcontractor's Application for Payment or AIA billing/Invoice
    • Attachment "B" - Subcontractor/Vendor Certification
    • Material Waivers - signed by vendors.
  3. What material waivers does Empire require to be submitted from it's subcontractors?
    1. We require waivers to be signed by all of your major vendor each month stating that you're paid in full up to the current pay application on the project.
    2. Major  vendors are defined as any company that you've hired to provide a total of $2,000 or more in supplies or services for a project.
