- Attachment "A" - Subcontractor's Application for Payment or AIA billing/Invoice
- Attachment "B" - Subcontractor/Vendor Certification
- Material Waivers - signed by vendors.
- What material waivers does Empire require to be submitted from it's subcontractors?
- We require waivers to be signed by all of your major vendor each month stating that you're paid in full up to the current pay application on the project.
- Major vendors are defined as any company that you've hired to provide a total of $2,000 or more in supplies or services for a project.
